In order to place business with us we will require a brokerage agreement along with a copy of your current brokers license and errors & omissions declarations page.

After you have completed and signed the agreement you can return it to us along with a copy of your license, e&o declaration and W-9 via fax or e-mail to:
After we receive the required information your account will be activated.

New Business Submission
In order to provide you with a quotation we must receive an application which will provide the information necessary for us to properly underwrite your account.
Please provide us with as much information as you can which will help us respond to you more quickly.
You can manually send us an application via e-mail or fax, or you can complete an on-line submission in our program. Any form of an insurance application is acceptable, but Acord applications are preferred and accepted by most of our insurance companies. Click on the Forms & Docs link and you will be able to find most common applications for your use. In some cases an additional supplement will be required. Your underwriter will let you know if this is required.
